Demographic change is regarded as one of the greatest challenges facing our society and the healthcare system. The steadily growing proportion of people with multiple co-morbidities and older people requires a committed expansion of geriatric networks and a thorough examination of the support needs of geriatric patients and clients. This can only be achieved if the specific needs, wishes and expectations of this client group are researched comprehensively and with a focus on quality - particularly in the area of inpatient medical, nursing and social care - and if support concepts are further developed (see Kraft 2008: 226). In this regard, clinical social workers can provide a wide range of support services which have a positive impact on the recovery process and improve and facilitate the daily lives of care home residents, provided that the services are tailored to the individual support needs of those affected. The focus should be on a biopsychosocial and salutogenic approach that emphasises the healthy aspects of the individual.
