HIV-1 infection is the leading cause of death in the Mwanza Region of Tanzania. This area is the site of the Tanzania-Netherlands research project on HIV infection in the region, for which the author was the project leader. The studies presented here cover the epidemiology of HIV-1 infection in adults in Mwanza Region.A population survey showed that HIV-1 infection affected 2.5% of the adult population in rural villages, 7.3% in roadside settlements and 11.8% in town, and was 1.5 times more common in women than in men. Results of the population survey were used to validate HIV-1 surveillance systems, and to show that HIV-1 infection was a risk factor for the development of tuberculosis and leprosy. In a factory cohort study, new HIV-1 infections occurred at a rate of 1.2% per year, and 62% of deaths were attributable to HIV-1 infection. Sexual partner change was common in this cohort and condom use extremely low. This book will be of particular interest to those involved in AIDS control and surveillance, and to epidemiologists concerned with the spread of HIV infection and its natural history.
