These composites are not real cases. They are not representations of identifiable individuals, families, or institutions. They are, however, real in the sense that matters most: every pattern they describe, every institutional failure they document, every moment of silence they record, is drawn from the authors' direct clinical and consultative observation of that pattern, that failure, that silence. The composites are designed to be recognizable to every professional in these systems - because what they describe is not exceptional. It is ordinary.
