Tacrolimus overdose in liver transplantation: plasmapheresis

Tacrolimus is most commonly used as a first-line immunosuppressant in solid organ transplantation. Few studies have reported on the benefits of plasma exchange in cases of tacrolimus overdose; tacrolimus is known for its high protein binding, extensive volume of distribution, high intra-erythrocytic concentration and hydrophobic nature. The aim of our study is to describe the benefits of plasma exchange in four liver transplant patients who presented with severe tacrolimus overdose. Our study is an observational, non-randomised, retrospective study conducted at the Toulouse University Hospital over a consecutive period of 40 months. Six liver transplant patients presented with severe tacrolimus overdose; of these, four underwent plasma exchange via a dialysis catheter.
